The first big question that most patients have is size. Implants don’t correlate to cup size, so Dr. Chasan’s experience can really help clients make the best choice for their frame. Just as important as size is shape.
We use two implants: Standard Silicone and Gummy Bear implant filled with highly cohesive silicone gel. Standard silicone implants produce a breast that has a round shape. Though they are very soft to the touch, they can look less natural. The Gummy Bear implant has more a tear drop shape that slopes to fuller bottomed breast. They are a bit firmer than the silicone, but not enough to be noticeable to the touch.
Implants are typically inserted beneath the pectoral muscle. The doctor normally makes an incision periareolar (bottom of areola), so there is little to no visible scarring after the procedure. Patients usually return to normal activities within a short period of time.

A breast lift is exactly as it sounds – a breast that has begun to sag or droop due to changes in weight, aging or pregnancy is uplifted. Depending on the circumstance, this is sometimes done in conjunction with breast augmentation surgery.
The procedure requires significant expertise because of the position of the incisions, which circle the nipple using one of three techniques: Donut, Lollipop, Short Scar, and Anchor. Dr. Chasan is known for his skill with minimally perceptible incisions
Once the incision is made, the doctor will adjust the underlying breast tissue higher. If needed, the nipple may be placed more appropriately, and the diameter of the areola trimmed to make it smaller and youthful.
Dr. Chasan will remove any excess skin and close the incision. Most patients are able to resume their normal activities within a week.
Breast Revisionary Surgery
Dr. Chasan is known in the San Diego area for correcting breast augmentation procedures where the outcomes weren’t acceptable. Sometimes this is as simple as removing a patient’s implants or repositioning implants that were poorly placed.
Other issues that may require surgery include capsular contracture, a condition when scar tissue develops and constricts the implant. Some surgeries are related to implants that shift after being inserted on top of the pectoral muscle.
But not all revisionary surgeries involve implants. Some other procedures can be the result of genetics, age or fluctuations in age, including reducing the size of the areola or there’s a variance in size between the breasts themselves.
Dr. Chasan’s experience is broad; he leads the field in revisionary surgery in the San Diego area and also travels the country to train other physicians in these techniques.
For women with breasts that are far too large for their frame, the issues go beyond cosmetic. Painful bra straps, back pain and embarrassment are the norm. Reducing the size of the breast isn’t just a matter of aesthetics, it’s a health issue.
Once the patient is sedated, Dr. Chasan will make an incision surrounding the areola, sometimes with a cut that goes down the breast to the crease. Next he will lift and adjust the underlying tissue, then remove any excess fat and skin. The nipple and areola are never surgically disconnected but sometimes their position is shifted as the contour of the breast is changed.
The doctor will close the incisions. There will be some minimal scarring that will fade over time and until them is covered by a bra or bathing suit top. Patients can expect to return to work within a week. The positive impact of the surgery is seen within the first week of the procedure.

Breast Reductive Augmentation
Women with disproportionately size breast want them reduced. breasts enter surgery wanting smaller breast, they also want more control over the shape. During the reduction process, an implant can be used to create a rounder, more attractive slope to the breast (called upper pole fullness.)
Dr. Chasan coined the term “Reductive Augmentation.” His expertise contributes to your quality of care and supports upper pole fullness.
During the breast reduction procedure described above, while removing tissue the doctor creates a pocket to insert a silicone or gummy bear implant. (The implant size is determined during your consultation.) The breast is replaced by the implant, improving shape.

Breast Reconstruction Surgery
Women who experience mastectomy can be candidates for reconstructive plastic surgery. In some cases, the reconstruction occurs directly after the breast surgery. Other patients may delay reconstruction until treatments are complete.
Reconstruction techniques to recreate the breast mound typically fall in two categories – implant based or flap-based which involves appropriating tissue from other areas of the body. Dr. Chasan will discuss your options and recommend what he considers the best path forward.
No matter what protocol you follow, reconstruction is typically not a single step procedure. Flap surgery normally has a more extensive recovery period and implants typically require tissue expansion.
It should be noted that reconstructive surgery is also an option for clients who have congenital breast deformities or a lumpectomy with a significant loss of tissue.
Tubular breasts are a rare genetic condition that causes the breasts to develop in a misshapen manner. A thick ridge of fibrous tissue forms around the base and the breasts protrude in a conical or tube-like shape. Though there is no physical health risk from the condition, the emotional discomfort is detrimental to self-esteem.
Tubular breasts are typically corrected with a combination of two techniques – breast augmentation and a breast lift. The doctor would make an incision on the bottom edge of the areola. From that vantage point, he can cut the restrictive tissue around the base of the breast. Once the breast is released, he can then prepare a pocket for the implant.
Tubular breasts tend to droop or sag, and the areola can be over-sized. Dr. Chasan will perform a lift to position the breasts more appropriately and modify the diameter of the areola if needed. (Please see Breast Lift procedure above for more detail.)
Breast Augmentation for Bodybuilders
Breasts sit on muscle, but they have none of their own. Their shape and size is determined by the amount of adipose (fatty) tissue, but female body builders tend to have little of it. Strict dietary protocols and rigorous exercise regiments can cause their breast mound to dwindle down to nothing.
Implants help their bodies regain feminine symmetry and provide balance to a muscular frame. Typically, Dr. Chasan will place implants beneath the pectoral muscle. The doctor makes a small incision periareolar to position the implant. Implants that are placed under the muscle tend to look more natural and make it easier to monitor breast health.
Some builders prefer to place the implants on top of the muscle. They have concerns about their ability to continuing to develop their pecs. But improvements in surgical technique and implant technology may have lessened some of those issues.
